What's in the Behavior Sticker Chart
Name and Dates
Fields for the child's name, the date started and the date the chart was full.
Behavior and Reward
Two ruled boxes: what I am working on, and what I earn when my chart is full.
Thirty Numbered Spaces
Thirty numbered squares in one frame, each with room for one sticker.
Print or Type
Type into the Word or Google Docs file. The 30 spaces are a picture, so print it. US Letter and A4.
How to Use the Behavior Sticker Chart
Name the Behavior
Write one behavior in the first box, as something the child does, such as raising a hand.
Pick the Reward
Let the child choose the reward, write it in the second box, and fill in the start date.
Add a Sticker
Each time the child shows the behavior, put a sticker in the next space and say what you saw.
Count Along
Count the filled spaces with the child now and then, so they can see how close they are.
Celebrate a Full Chart
When all 30 spaces are full, write the date in Chart Full On and give the reward right away.
