List the Groups
On page 1, name each group and write in the students who belong to it for the week.
Give Each Group a Skill
Write one focus skill per group, based on what you saw in your last check.
Plan the Activity
Note the activity and materials for each group, so you can set the stations out ahead.
Plan One Group Lesson
On page 2, write a warm-up, a teaching point and guided practice for a single group.
Record What You See
During the lesson, jot what each student did, then write the next step for them.
