Bio Poem Template Example

Maya's bio poem about herself, written line by line beside the pattern.

Bio poem filled in by a 5th grader named Maya, one line per row of the pattern: her name, four traits, sister of Leo, lover of soccer and her dog, what she feels, needs, fears, gives and would like to see, where she lives, and her last name.

What's in the Bio Poem Template

Eleven Lines of the Pattern

A table lists each line number and the words it asks for, from first name to last name.

Space to Finish Each Line

A My Bio Poem column sits beside each prompt, and it reads as the finished poem.

Name, Subject and Date

Fields for the writer, who the poem is about and the date.

Editable Versions

Type into the Word, Google Docs or PDF file, or print it to fill in by hand. US Letter or A4.

How to Use the Bio Poem Template

  1. Choose the Subject

Students write whether the poem is about themselves, a character or a person they are studying in the field.

  2. Start with the Name

Line 1 is the first name. Line 2 is four words that describe the subject, written on one line.

  3. Work Down the Pattern

Students write relative of, lover of, then who feels, needs, fears, gives and would like to see.

  4. Finish the Poem

Line 10 is where the subject lives, and line 11 is the last name. Students then read it aloud.

  5. Share the Poems

Post the poems on the wall or put them in a class book, and have students guess a classmate from each one.
